## What It Is

FOXO4-DRI is a D-retro-inverso peptide designed to disrupt the interaction between FOXO4 and p53 in senescent cells. By blocking this interaction, p53 is released to trigger apoptosis selectively in senescent cells while sparing healthy cells. A landmark 2017 mouse study showed improved fitness, fur density, and kidney function in aged mice. It represents a targeted approach to cellular senescence. In 2026 rankings of evidence-based anti-aging peptides, FOXO4-DRI entered the top 10 for the first time, reflecting growing preclinical evidence for selective senescent cell clearance. The senolytic approach — removing damaged cells rather than repairing them — is gaining traction as a complementary strategy alongside mitochondrial peptides (SS-31) and metabolic regulators (MOTS-c). As of early 2026, researchers continue to work on optimizing FOXO4-DRI and related senolytic peptides. Cleara Biotech, the company formed to commercialize the original Baar et al. research, remains a going preclinical concern focused on the FOXO4-p53 interaction as a therapeutic target. Side-by-side comparisons from the original study showed FOXO4-DRI cleared 70% of senescent cells with zero platelet count reduction, while navitoclax at equivalent senolytic efficacy dropped platelet counts by 40–60% — highlighting the peptide's selectivity advantage over small-molecule senolytics. No human trials have been initiated as of May 2026. Expanded in vitro work in 2026 using human chondrocytes confirmed selective removal of senescent cells while preserving healthy cell populations, supporting the peptide's potential for osteoarthritis and other joint-related degenerative conditions.
