## Medicare GLP-1 Bridge: $50/Month Starting July 1, 2026

The Centers for Medicare & Medicaid Services (CMS) will launch the Medicare GLP-1 Bridge on July 1, 2026 — a time-limited demonstration program giving eligible Medicare Part D beneficiaries access to FDA-approved GLP-1 weight-loss medications at a flat $50-per-month copayment. The program runs through December 31, 2027. Three medications are covered: Foundayo (orforglipron), Wegovy injection, Wegovy tablets (oral semaglutide), and Zepbound (tirzepatide KwikPen). The $50 copayment will not count toward a beneficiary's Part D deductible or the $2,100 annual out-of-pocket maximum (rising to $2,400 in 2027), and will remain flat regardless of which benefit phase the beneficiary is in when filling their prescription. The program operates outside the standard Part D coverage flow, meaning participating Part D plans do not carry financial risk for these claims. CMS estimates the program could benefit millions of the approximately 67 million Medicare enrollees who qualify based on BMI and related health criteria. For context, obesity affects roughly 41% of adults aged 60 and older, and this age group has historically faced the highest cost barriers to GLP-1 therapy — with monthly list prices for Wegovy and Zepbound ranging from $1,300 to $1,700 before rebates or insurance.

## Who Qualifies and How It Works

To be eligible for the Medicare GLP-1 Bridge, beneficiaries must be enrolled in a Medicare Part D plan and meet the FDA-approved indications for one of the three covered drugs: obesity (BMI ≥30) or overweight (BMI ≥27) with at least one weight-related comorbidity such as hypertension, type 2 diabetes, dyslipidemia, or obstructive sleep apnea. Importantly, beneficiaries must also be enrolled in a participating lifestyle intervention program — either CMS's BALANCE (Better Approaches to Lifestyle and Nutrition for Comprehensive hEalth) Model or another qualifying program. The BALANCE model is a voluntary program running through 2030 that pairs GLP-1 access with intensive lifestyle counseling, behavioral health support, and nutritional guidance at no additional cost. This requirement reflects CMS's goal of pairing pharmacotherapy with behavioral support to maximize long-term outcomes and reduce recidivism. To access the $50 copayment, beneficiaries work with their prescriber to confirm eligibility and enroll in a qualifying lifestyle program, after which the reduced copayment is applied automatically at the pharmacy.

## CagriSema FDA Review: October 2026 Decision Expected

Novo Nordisk's CagriSema — a fixed-dose once-weekly combination of cagrilintide 2.4 mg (amylin analogue) and semaglutide 2.4 mg (GLP-1 RA) — remains under FDA review following its NDA submission on December 18, 2025. Under a standard 10-month review clock, the PDUFA action date would land around October 2026. No complete response letter, clinical hold, or advisory committee meeting request has been announced, suggesting the review is progressing on schedule. In the Phase 3 REDEFINE 1 trial, CagriSema produced 22.7% weight loss at 68 weeks versus 2.4% for placebo, with 91.9% of participants achieving ≥5% body weight reduction. ADA 2026 (June 5–8, New Orleans) featured new CagriSema data from the REIMAGINE program in type 2 diabetes, confirming its cardiometabolic benefit profile. If approved on time, CagriSema would enter the market in October or November 2026 as the first amylin/GLP-1 combination therapy — a mechanistically distinct approach from existing GLP-1 or dual GLP-1/GIP agents. Its approval would likely require re-evaluation of formulary positioning across major PBMs, which already made significant changes following CVS Caremark's June 2026 formulary reset.

## Retatrutide NDA: On Track for Q4 2026 Filing

Eli Lilly's retatrutide — a triple agonist activating GLP-1, GIP, and glucagon receptors simultaneously — now has all primary efficacy data required for an NDA submission, which remains on track for Q4 2026. The pivotal TRIUMPH-1 trial (n=2,339, 80 weeks) confirmed 28.3% average weight loss at the 12 mg dose with 45.3% of participants achieving ≥30% loss — the highest efficacy numbers for a commercial obesity drug to date. TRIUMPH-4 (obesity with osteoarthritis) and TRANSCEND-T2D-1 (type 2 diabetes) also reported positive results at ADA 2026. Under a standard FDA review timeline, NDA submission in Q4 2026 would yield a PDUFA date in late 2027 (October–December), or Q1 2028 under a 12-month standard review. Priority review designation (6 months) remains possible but has not been confirmed. If approved, retatrutide would be the first triple agonist to reach commercial launch, directly competing with Zepbound (tirzepatide), CagriSema, and Wegovy in a market that IQVIA projects could reach $130 billion by 2030. Eli Lilly has not announced a brand name for retatrutide in the US market.

## PCAC July 23–24 Countdown: 43 Days to the Most Consequential Research Peptide Vote in Years

The FDA's Pharmacy Compounding Advisory Committee (PCAC) meeting on July 23–24, 2026 is now 43 days away. Seven peptides are scheduled for formal review: BPC-157 (free base and acetate), TB-500 (Thymosin Beta-4 fragment), KPV, MOTS-c, Semax, Epitalon, and Emideltide (DSIP). The committee will vote on whether each peptide should be added to the 503A Bulks List, which would permit compounding pharmacies to prepare them with individual prescriptions. The public comment docket (FDA-2025-N-6895) remains open; comments submitted by July 9, 2026 will be presented to the committee before the vote. A favorable PCAC vote is not the final step — formal rulemaking would still be required before compounding pharmacies can legally prepare listed substances — but it would represent the first positive formal regulatory action for any of these peptides in the US since the 2023 restrictions took effect. The research peptide community has been tracking this meeting closely since the FDA's April 15, 2026 announcement that it removed 12 peptides from Category 2 (significant safety concerns) following nomination withdrawals. For researchers and clinicians, the July meeting represents the highest-stakes regulatory event in the peptide space in at least three years, and its outcome will shape compounding pharmacy practice well into 2027.
