## The adherence problem inclisiran was built to solve

Lowering LDL cholesterol is one of the most reliable ways to reduce heart attacks and strokes, but the medicines that do it - statins and, more recently, injectable PCSK9 antibodies - only work if people keep taking them. Daily pills get skipped; injections every two to four weeks get missed. Inclisiran (brand name Leqvio) attacks that adherence problem head-on. It is a small interfering RNA (siRNA) given as a subcutaneous injection just twice a year after a loading dose, and it lowers LDL cholesterol by roughly half by silencing a single liver protein: PCSK9. It was the first RNA-interference medicine approved for a common chronic cardiovascular condition.

## What PCSK9 does, and why silencing it lowers LDL

The liver clears LDL cholesterol from the blood using LDL receptors on its surface, which grab LDL particles and pull them inside. PCSK9 (proprotein convertase subtilisin/kexin type 9) is a protein the liver makes that binds those receptors and sends them to be destroyed, so the more PCSK9 you make, the fewer LDL receptors you keep and the higher your LDL rises. Turn PCSK9 down and the opposite happens: more LDL receptors survive and recycle, and LDL cholesterol falls. Human genetics back the target - people born with loss-of-function PCSK9 variants run low LDL and have less coronary disease. Inclisiran is designed to reproduce that low-PCSK9 state on demand.

## How inclisiran is built: GalNAc-siRNA

Inclisiran is a double-stranded RNA attached to a triantennary GalNAc (N-acetylgalactosamine) tag - a sugar that binds a receptor found almost only on liver cells. That tag carries the siRNA straight into hepatocytes, where it hijacks the cell's natural RNA-interference machinery to find and destroy the messenger RNA for PCSK9 before the protein can be made. Because the silencing is catalytic and long-lasting, one injection keeps PCSK9 - and LDL cholesterol - suppressed for months. That is what lets inclisiran be dosed on day 0, again at three months, and then only once every six months. The molecule was discovered by Alnylam, developed by The Medicines Company, and came to Novartis when it acquired that company in 2020.

## Antibody vs siRNA: two ways to hit PCSK9

Inclisiran is not the first PCSK9 medicine. The monoclonal antibodies evolocumab and alirocumab, approved years earlier, also target PCSK9 - but they work downstream, binding and neutralizing PCSK9 protein already circulating in the blood, and must be injected every two to four weeks. Inclisiran works upstream, stopping the liver from producing PCSK9 in the first place. The practical payoff is dosing frequency: a couple of injections a year instead of one or two a month. The trade-off is that because RNA silencing is deep and durable, its effect cannot be quickly switched off once a dose is given.

## ORION-10 and ORION-11: the Phase 3 LDL test

The pivotal evidence came from three Phase 3 trials. ORION-10 (in U.S. patients with atherosclerotic cardiovascular disease) and ORION-11 (in European and South African patients with ASCVD or risk equivalents), published together in the New England Journal of Medicine in 2020, enrolled 3,178 adults whose LDL cholesterol was still at or above 70 mg/dL despite maximally tolerated statins. Participants received inclisiran 284 mg or placebo on day 1, day 90, and every six months after. At day 510, inclisiran lowered LDL cholesterol by about 52% in ORION-10 and about 50% in ORION-11 versus placebo, with reductions holding steady across the dosing interval. A third trial, ORION-9, extended the benefit to people with inherited heterozygous familial hypercholesterolemia, cutting LDL by roughly 48%.

## Durability and safety

Because the whole appeal of inclisiran is infrequent dosing, durability matters. The four-year open-label ORION-3 extension of the earlier ORION-1 study, reported in The Lancet Diabetes & Endocrinology in 2022, showed LDL reductions of around 45% maintained over four years of twice-yearly dosing. On safety, a 2023 pooled analysis of seven trials in the Journal of the American College of Cardiology found overall adverse events similar to placebo; the main difference was more frequent injection-site reactions - redness, pain or rash - that were mostly mild and short-lived, with no signal of liver, muscle, kidney or platelet harm.

## Approvals - and the 2025 move to first-line

Inclisiran reached the market as Leqvio: the European Union granted marketing authorization on December 9, 2020, and the U.S. FDA approved it on December 22, 2021, initially as an add-on to maximally tolerated statins for adults with ASCVD or heterozygous familial hypercholesterolemia. On July 31, 2025, the FDA updated the U.S. label to allow first-line monotherapy, removing the requirement that inclisiran be used on top of a statin and refocusing the indication on lowering LDL cholesterol in adults with hypercholesterolemia. That change reflects how confident regulators have become in the LDL-lowering effect of PCSK9-targeted therapy.

## The test that still matters: cardiovascular outcomes

For all its LDL lowering, inclisiran has not yet been shown to prevent heart attacks and strokes - that is a separate, harder question. A large outcomes program is running to answer it. VICTORION-2-PREVENT (NCT05030428) is testing whether inclisiran added to statins reduces 3-point major adverse cardiovascular events (cardiovascular death, non-fatal heart attack and non-fatal ischemic stroke) in adults with established ASCVD, with primary completion expected around 2027; companion trials include VICTORION-1-PREVENT in high-risk primary prevention and VICTORION-INTERVENTION. Until those read out, inclisiran's benefit rests on the strong and well-established link between lower LDL and lower risk, not on its own outcomes data.

## Where inclisiran sits in the RNAi wave

Inclisiran is the proof-of-concept the rest of the field builds on. It showed that a GalNAc-siRNA could be turned into a convenient, approved, chronic cardiovascular medicine - and that template now runs through a whole generation of investigational siRNAs aimed at other lipid targets: ANGPTL3 (zodasiran, solbinsiran), apoC-III (plozasiran, olezarsen), Lp(a) (olpasiran, lepodisiran, zerlasiran, pelacarsen) and even blood pressure via angiotensinogen (zilebesiran). Inclisiran is the one that is already approved and in pharmacies, which is why it anchors the category and sets the bar the newer agents are measured against.

## What to keep in mind

Inclisiran is an approved prescription medicine given by or under the supervision of a healthcare professional - not a supplement or research chemical, and anything sold as 'inclisiran' or 'Leqvio' outside a pharmacy or clinic is unverified and unsafe. It reliably lowers LDL cholesterol with a safety profile close to placebo apart from mild injection-site reactions, but a proven reduction in heart attacks and strokes is still pending from the VICTORION trials, and its long-lasting effect cannot be quickly reversed once dosed.
